Integrated care in the context of health insurance refers to the coordination of benefits for dually eligible individuals. This coordination is primarily accomplished through private plans that pay for and deliver an individual’s Medicare and Medicaid services. Examples of these private plans include Dual-eligible Special Needs Plans (D-SNPs), Fully Integrated Dual-Eligible Special Needs Plans (FIDE SNPs), Highly Integrated Dual-Eligible Special Needs Plans (HIDE SNPs), and Medicare-Medicaid Plans (MMPs).
