When can I join a SNP?

If you meet the SNP’s eligibility requirements, you can join the plan during the regular Medicare health plan enrollment periods.

    For more information about when you can enroll in or change your Medicare health plan, click on the link in the GO TO box.

In addition, you will also be eligible for a Special Enrollment Period (SEP) to join a SNP if you

  • have Medicare and become eligible for Medicaid. The enrollment period lasts as long as you have Medicare and Medicaid.

  • have a severe, disabling or chronic condition. You can enroll in a SNP at any time as long as you have the condition.

  • are entering an institution that qualifies you for SNP coverage, or develop the need for an institutional level of care.
